Toward Sustainable Health Management through Intelligent Digital Care



Dr. Yeonsoo Jang has pursued a sustained research agenda in self‑management for patients with liver disease since 2011, progressively shifting toward interventions that promote durable health behavior change and improvements in quality of life. Building on this foundation, she now directs a practice‑oriented program of research that places digital health at the core of nursing intervention development.


From 2026 to 2030, she is leading the research project “PRIME‑aiLiver: Development and Evaluation of an Agent‑Based Digital Personalized Integrated Management Program for Patients with Metabolic Dysfunction‑Related Fatty Liver Disease” (Core Research Grant Type B), funded by the National Research Foundation of Korea. PRIME‑aiLiver® aims to operationalize AI and agent‑based technologies into a nurse‑delivered, digitally enabled service that supports personalized and sustained self‑management.

The research team will develop an AI‑driven risk‑stratification algorithm and an agent‑based digital intervention platform that synthesizes heterogeneous real‑world clinical data to deliver tailored prompts, educational modules, and actionable care plans to patients and their families. Crucially, the project frames these technologies as enablers of nursing practice: the digital infrastructure augments clinical reasoning, prioritizes intervention targets, and facilitates scalable, family‑centered support pathways in routine care settings. Planned effectiveness evaluations will iteratively refine the program and assess its impact on healthcare access, long‑term adherence, self‑care capacity, health behaviors, and patient‑reported outcomes.


By embedding digital health within a nursing intervention service model, Dr. Jang’s work seeks to demonstrate a replicable approach for integrating AI‑enabled personalization into chronic disease management, thereby advancing both the science of nursing interventions and the practical delivery of sustainable, equitable care for individuals with chronic diseases.
